Mindseye Escapes LLC — Photo & Media Release Agreement

By granting this release, I hereby authorize Mindseye Escapes LLC and its designees to photograph, film, record, and otherwise capture my image, likeness, voice, and appearance during the retreat experience (collectively, "Content").

I grant Mindseye Escapes LLC a non-exclusive, royalty-free, worldwide, perpetual license to use, reproduce, distribute, display, and publish such Content in any medium now known or hereafter developed, including but not limited to social media, websites, marketing materials, editorial features, and promotional campaigns, without further compensation or notification to me.

I understand that my identity may or may not be disclosed in connection with any such use, and I waive any right to inspect or approve the finished product. I release Mindseye Escapes LLC, its officers, employees, agents, and assigns from any and all claims arising out of or in connection with the use of such Content.

This release does not apply to Content captured in private accommodations or spaces designated as photography-free by retreat staff. Declining this release will not affect your retreat experience in any way.

This agreement is governed by the laws of the State of California, without regard to conflict of law principles.

I do not consent to the use of my image or likeness in any Mindseye Escapes marketing or promotional materials.

Data Release & Confirmation

I certify that all flight details, shoe sizing, and contact data submitted above is completely accurate. I acknowledge that these choices lock into the master operations sheet for Mindseye Escapes LLC, Solaz Resort, and Platinum Cabo exactly thirty (30) days prior to departure, and that any modifications inside this window are subject to administrative operational holds.
